The Renal Coordinator Unit provides education, empowerment and assistance to patients with kidney disease. We believe that well-informed and supported patients achieve better health outcomes. Our comprehensive support services equip both patients and caregivers with the necessary tools and knowledge to manage kidney disease with confidence.

Education & Outreach

Our dedicated coordinators deliver educational programmes covering renal health, treatment options and self-care techniques. We regularly host forums, workshops and support groups to enhance patient care and understanding.

 

Advanced Practice Nurses

Our Centre houses an exceptional team of Advanced Practice Nurses (APNs) and specialist nurses who deliver excellent renal care across multiple specialties.

Our nursing team provides specialised services encompassing Interventional Nephrology, Renal Replacement Therapies, Acute Kidney Injury/Disease Management, Telemedicine consultations, and innovative extracorporeal therapies. Working autonomously, our APNs and specialist nurses manage renal cases, conduct medication prescriptions and titration, and deliver evidence-based care through dedicated clinics. These specialised services include Chronic Kidney Disease (CKD) management, Peritoneal Dialysis, Critical Care Nephrology, Transplant Care, and Renal Supportive Care.

Our nurse clinicians are at the forefront of advancing nursing practice through active engagement in research and quality improvement initiatives. As respected educators and mentors, they contribute to global knowledge exchange by participating in international conferences, conducting training programmes, and hosting clinical attachments for healthcare professionals worldwide.

Renal & Transplant Coordinators

Serve as key contact points for navigating the patient journey—arranging appointments, preparing for dialysis or transplant, and offering psychosocial support.

Healthcare Financing

The “S+3Ms” is Singapore’s healthcare financing framework designed to assist all Singaporean Citizens and Permanent Residents in managing medical expenses at public healthcare institutions. It comprises four key components:

Advance Care Planning (ACP) is a series of voluntary conversation where you plan for your future healthcare decisions. It involves:

  • Discussing your personal values and beliefs
  • Expressing your priorities and  healthcare preferences
  • Appointing a Nominated Healthcare Spokesperson (NHS) who will speak on your behalf
  • Having conversations with your loved ones and healthcare team

By expressing your wishes ahead of time, it can guide your loved ones and healthcare team to provide care and treatment that aligns to your values and preferences when you are unable to communicate.
