Supporting Care Across the Community
General Practitioners (GPs) play a key role in meeting the majority of Singapore’s primary care needs. Through close collaboration with community GPs, Singapore General Hospital (SGH) supports timely access to specialist care while enabling continuity of care across settings.
Our Services
GPs Referrals via Corporate Insurance Plans
SGH also facilitates direct access to outpatient specialist care for patients whose treatment is covered under their corporate insurance employee benefit plans.
GPs may coordinate referrals in line with coverage requirements and insurer arrangements to support timely access to care.
CHAS Accredited GPs Referrals to SGH Specialist Outpatient Clinics (SOCs)
GPs accredited under the Community Health Assist Scheme (CHAS) may refer eligible CHAS patients to SGH Specialist Outpatient Clinics (SOCs) for subsidised care.
The following conditions apply for subsidised CHAS referrals:
- Referrals must be unnamed (i.e. not directed to a specific specialist)
- Referrals must be for conditions covered under CHAS
- CHAS‑accredited GPs are unable to make direct referrals for specialised investigations (e.g. laboratory or radiology services) or Allied Health services.

Documents for Patients to bring for SOC Appointments:
- NRIC
- Health Assist Card / PA Card / existing CHAS card
- CHAS Subsidised Referral Cover Note issued by the referring GP
Patients must present the above documents to enjoy subsidised charges.

SingHealth DOT Primary Care Network
As part of national efforts to strengthen primary care, the Ministry of Health introduced the Primary Care Network (PCN) scheme, which supports private GP clinics in delivering holistic, team based care.
Under the PCN scheme:
- Participating GPs receive funding and administrative support
- Patient outcomes and chronic conditions are tracked more systematically
- GPs are supported by dedicated nursing, allied health, and administrative teams
- A chronic disease registry enables closer monitoring of patient health
This model supports more coordinated and integrated care between primary and specialist services.
SingHealth DOT Right-Siting Programme
The SingHealth Delivering On Target (DOT) Programme supports the right siting of patients with stable chronic conditions from Specialist Outpatient Clinics at SGH and the National Heart Centre Singapore to partner GPs.
This programme enables:
- Care to be delivered closer to patients’ homes
- Better continuity and long‑term management in the community
- Appropriate use of specialist services for patients who require them
