The SingHealth DOT Primary Care Network (DOT PCN) is a collaborative initiative comprising a network of DOT PCN GP clinics, supported by a dedicated team of nurses, administrative staff and primary care coordinators. Together, we strive to deliver the best care to our patients.

What services are available for our DOT PCN GP partners?

Nurse counselling at Yim Clinic & Surgery

Our team is here to help our GP partners provide excellent care for patients with chronic diseases. We offer ancillary services and nurse counselling which include:

  • Diabetic Retinopathy (DRP)
  • Diabetic Foot Screening (DFS)
  • Nurse Counselling (NC)
  • Spirometry

5th Anniversary of SingHealth DOT PCN

We also hold quarterly review sessions for our GP partners. These sessions include Continuing Medical Education (CME) activities to ensure everyone stays updated on the latest information and operational updates in line with Ministry of Health (MOH) guidelines.
