Telecare@SGH

Telecare@SGH team consists of TeleCarers (Registered Nurses and Care Coordinator Associates) who provide telehealth services, such as tele-monitoring and tele-education via phone or video consultation. 

We aim to support patients’ recovery at home by working with both patients and caregivers. 

Our Services

✔ Post-discharge tele-monitoring to track patient progress and compliance to treatment plan(s)

✔ Early identification of potential health issues and care escalation 

✔ Tele-education to empower patients and caregivers in chronic disease self-management

✔ Referrals to appropriate community service providers for continued care

TeleCare@SGH’s services are available to patients who:

• Were recently discharged from SGH Accident & Emergency and
• Reside within the following Southeast precincts: Bukit Merah, Chinatown, Kampong Glam-Kallang, Katong, Rochor, Telok Blangah, Tiong Bahru
