Please email the completed form to giftstosgh@sgh.com.sg. For GIRO deduction, the bank requires the original hardcopy donation form for processing.

Staff Donation Forms and drop-off box are also available at Human Resource Office, SingHealth Tower Level 3.

If you have donated using PayNow QR/SingTel Dash/AXS station, please email giftstosgh@sgh.com.sg with the screenshot of your transaction and your employee number for reconciliation purposes.    

All eligible donations will enjoy 250% tax deduction. The details of your donation will be automatically included in the donor’s IRAS tax assessment and tax deductible receipt will be issued only upon request.
