Community Nursing Volunteer Support Group

As healthcare needs become more complex, caring for seniors goes beyond hospital settings and into the community. Many seniors benefit from ongoing support that helps them stay healthy, independent, and connected to the people around them.

The Community Nursing Support Group is part of the SingHealth Southeast Community Nursing initiative, which began as a pilot programme on 28 February 2018. The programme was established to support population health and to shift care beyond acute hospital settings into the community. Through geographically based community nursing teams, the initiative focuses on health promotion, early intervention, and reducing or delaying the need for hospital care.

The support group was initiated to meet the growing demand for volunteers to engage, empower, and support seniors living in the community. By working alongside the SGH Community Nursing Team, volunteers play an important role in supporting the physical, mental, and social well‑being of seniors, while also strengthening community connections.

How we help seniors in the community

• Support community outreach programmes organised by SGH Community Nursing, such as falls prevention initiatives
• Engage seniors through regular interactions to promote physical, mental, and social well‑being
• Encourage seniors to feel connected to their community by spending quality time with them and recognising them as individuals with unique needs and experiences
• Assist with ad‑hoc volunteer activities when community needs arise

Volunteers work alongside geographically based community nursing teams who provide services such as geriatric and health assessments, health coaching for disease prevention, chronic disease monitoring, self‑management education, medication self‑management support, and case referral and coordination.

As of March 2026, SGH Community Nursing operates 52 Community Health Posts across seven precincts—Bukit Merah, Chinatown, Telok Blangah, Tiong Bahru, Katong, Kallang, Kampong Glam, and Rochor. This community‑based approach allows for better understanding of local population needs, improved accessibility, closer collaboration with partners, and stronger capability‑building for health and social care personnel.

Participation in the support group also provides volunteers with opportunities to learn, grow, and develop leadership qualities through hands‑on community engagement.
