For prompt evaluation and management of your patients by our specialists, SGH GP Network offer direct access to our specialist outpatient clinics and services. GPs registered under Community Health Assist Scheme (CHAS) can refer CHAS patients to SGH Specialist Outpatient Clinics (SOCs) for subsidised treatment. The following applies for subsidised referrals:
Referral must be unnamed (e.g. GPs cannot make referral to specific specialists).
Referrals can be made for conditions covered under CHAS.
CHAS accredited GPs cannot make direct referrals to specialised investigations (e.g. laboratory/radiology) or Allied Health.
Patients must remember to bring along the following for their SOC appointment to enjoy the subsidised charges.

In Singapore, GP clinics serve about 80 per cent of the population’s primary care needs. To support continuous care, SingHealth works closely with GPs across the continuum to enable patients to be cared for adequately in the community.
As part of national efforts to strengthen primary care, the Ministry of Health introduced the Primary Care Network (PCN) scheme to encourage private GP clinics to organize themselves into networks that support holistic and team-based care in the community.
Under the PCN scheme, funding and administrative support are provided to help participating GPs to better track outcomes and monitor their patients’ health conditions more closely. Participating GPs are supported by a team of dedicated nursing, allied health and administrative staff and a chronic disease registry.
