Family Medicine Continuing Care

The Family Medicine and Continuing Care (FMCC) Department at Singapore General Hospital envisions a healthcare ecosystem that champions the needs of a rapidly ageing population while fostering a healthier Singapore. Our mission is rooted in driving transitional care services, leveraging on new care models and interventions to ensure smooth transition of care for patients within and beyond the confines of our hospital, contributing to a healthier and more resilient society in Singapore.

Clinical Services

FMCC clinical services cover the entire spectrum of the care continuum.

The aim of the hospital-based initiatives is to reduce readmissions and length of hospitalisation. Our hospital-based initiatives are:

  • Inpatient consultative services
    • Inpatient blue letter service that facilitates care coordination, discharge care planning and chronic disease management.
  • Mobile Inpatient Care at Home (SGH@Home)
    • Home hospitalisation service that offers clinically suitable patients the option of being cared for in the comfort of their own homes, instead of a hospital ward.
  • Transitional Home Care Services
    • Urgent Care Response Team (UCRT): Transitional care service that offers urgent transitional care/ supported discharge, community assessment, triage and management of sub-acute cases.
    • Hospital to Home (H2H): Transitional care service that offers supported discharge from acute hospital for post-acute patients and community chronic disease management.

  • Specialist Outpatient Clinics (SOC)
    • Ambulatory Family Medicine Continuing Care Clinics
  • Health Assessment Centre (HAC)
    • Health Screening Service
