Family Medicine Continuing Care

At FMCC, we seek to coordinate care when patients are discharged from hospital to home, especially patients with complex care needs. Care integration and continuing care in the community forms an integral part of our daily roles and responsibilities.

Teaching

The FMCC department partakes in a large variety of undergraduate and post graduate family medicine training. This includes:

• Yong Loo Lin- National University of Singapore undergraduate Family Medicine programs
• Duke-NUS post graduate Medical School
• Family Medicine Residency training
• Medical elective postings (local and overseas)

Research
