Embracing Ageing - Collaboration between Hospital & Community Pharmacists

The SGH-UIC Ambulatory Care Course: Embracing Ageing - Collaboration between Hospital & Community Pharmacists will discuss the role of pharmacists in geriatric care across the continuum of care (hospital & aged care facilities). It enables participants to recognize the various geriatric assessment tools in clinical assessment of the older adults. Participants will also identify challenges in managing psychiatric symptoms in old adults and design appropriate pharmacotherapeutic plan to ensure safe and effective use of medications in the geriatric population and explore opportunities for health-related needs in the ageing population such as palliative care and vaccinations.
