PGAHI Terms & Conditions*


Personal Data Protection Act (PDPA)


By registering for PGAHI's programmes, you agree to the following clauses:



I consent to Singapore General Hospital and its related corporations (collectively ‘SingHealth’), their agents and SingHealth’s authorised service providers collecting, using, disclosing and/or processing my personal data, in order to send me marketing materials, etc.



I confirm and agree that my consents granted herein do not supersede or replace any other consents which I may have previously provided to SingHealth in respect of my personal data, and are additional to any rights which SingHealth may have at law to collect, use or disclose my personal data.



By providing the information set out in this form and submitting the same to you, I confirm that I have read, understood and consent to the SingHealth Data Protection Policy, a copy of which is available at Hard copies are also available on request.








I declare that the information provided for course registration is true and accurate to the best of my knowledge. Submission of any false information will render my application void with no refund of registration fee. Submission of my professional certification or qualifications for auditing purpose may be required.








Registration will only be confirmed upon receiving receipt of confirmation email.






Refund Policy for Cancellation/Withdrawal/Absenteeism







Any cancellation or replacement request initiated by confirmed registrant/participant must be conveyed to PGAHI in writing.




No cancellation fee is applied if request is received on 21 calendar days or more before course/programme start date.




A cancellation fee of 50% of course fee will be charged if cancellation request is less than 21 calendar days from the course/programme start date.



Absenteeism and Replacement Classes



There is no make-up class and refund of course fee for partially present or no-show.



Programme Cancellation/ Postponement



PGAHI reserves the right to cancel or postpone any programme due to unforeseen circumstances.



In the event that a course is cancelled and course fee has been paid, PGAHI will only refund the fee received to the participants. For foreign participants, there will be no refund of accommodation, air-ticket and other travel-related expenses.



Request for Postponement/ Transfer of Course



In the event if a participant requests to postpone his/her registration or transfer to a different course, it will be considered as a new registration. Such requests made will also be subjected to the cancellation policy (refer to Point 4.1) based on the initial registration.






Course Certificates



Participants must attend at least 75% of the sessions at the workshop in order to receive the Certificate of Attendance awarded by SGH PGAHI.



Reprinting of certificate is chargeable at $30 (inclusive of GST) per reprint.






Accrual of Continuing Professional Education (CPE) Points



For lectures approved for CPE points accrual, participants who arrive 15 minutes after the stipulated lecture start time will not be accrued with the CPE points.



For accrual of CPE points that involve online (synchronous)/ off-line (asynchronous) education programmes, participants may be required to submit their application of CPE points directly to their respective council bodies.








I hereby unconditionally and irrevocably agree:



to indemnify forthwith on demand and hold SGH and each of the other SingHealth Companies and their respective officers, directors, employees, consultants, agents, representatives and professional advisers (collectively the “SingHealth Indemnitees”) harmless against any and all losses, damages, liabilities, demands, claims, actions, proceedings (including all direct, indirect, consequential, special, incidental or punitive damages or losses), costs (including legal costs on an indemnity basis) and expenses, fees, fines, penalties, interest and any form of economic loss (including loss of reputation) whatsoever and howsoever arising from or in connection with:




any act, omission, default or negligence; and/or




any breach of any law or any policy, undertaking or rule whatsoever of SGH or any of the other SingHealth Companies;



by me or any person acting on my behalf or under my supervision including that during the course of or relating to the SGH Studies, access to or use of any of their respective premises; and



that any of the other SingHealth Indemnitees may each in its own right call on the indemnity and enforce the rights provided for in paragraph (a) above.








Copying of any forms such as recording, reproducing, photo-taking; adaption or any other use of all or any part of the training materials without permission is strictly prohibited.





*PGAHI reserves the right to change the terms and conditions at any time. We also reserve the right to grant and refuse refund at its sole absolute discretion.
