Fewer appointments and more remote monitoring for stable Axial Spondyloarthritis patients

If you’re living with Axial Spondyloarthritis (axSpA) and your condition is stable, you may still need regular check-ins to monitor for flare-ups and any side effects from medication. We understand that frequent clinic visits can be time-consuming, especially when your schedule is already packed.

To make your care more convenient and reduce the need for in-person appointments, our team at SGH, led by Dr Kwan in collaboration with SingHealth AIA, has developed the Automated Monitoring Scheme (AMS).

This new system uses your blood test results and self-reported symptoms (PROMs) to track your condition remotely. You can stay on top of your health with less disruption to your daily life.

This system automatically schedules in-person appointments to a later time, up to one year.

And remotely monitors symptoms 4 monthly. (Hermans et al)

 

A virtual follow-up appointment will be booked after 4 months if

  • Blood test are normal
  • PROMs results are within threshold

 

Physical clinic / Urgent Care clinic (UCC) is required only in the following scenarios:

  • Patient did not do any PROMs OR took any blood tests
  • Exceed threshold for bloods OR PROMs
  • Patient keen for physical review
