SGH Community Nursing Programme provides a wide range of services for residents who are pre-frail and frail, have chronic diseases, or require complex care, in the five Communities of Care:

Bukit Merah
Chinatown
Telok Blangah
Tiong Bahru
Katong

The nurses from SGH are stationed in the community to provide accessible healthcare to residents and conduct public outreach programmes, such as fall prevention programme and health talks, focusing on preventive health and management of chronic health conditions.
