What is my Patient Classification?

You are a private patient:

  • If you specify the name of the specialist you wish to see. OR
  • If you are referred by a private practitioner or hospital. OR
  • If you are referred by a restructured hospital where you are a private patient. OR
  • If you are a foreign resident or non-resident of Singapore.


You are a subsidised patient:

  • If you are a Singapore Citizen or Singapore Permanent Resident and referred by a polyclinic. OR
  • If you are referred by a restructured hospital where you are a subsidised patient. AND
  • You do not specify the name of specialist you wish to see.

Subsidised patient will be charged subsidised rates.



Preparing for appointment

If you have been requested to perform any tests or scans, please complete them before your appointment.

If you are required to fast before your appointment, please follow the specific instructions that were shared with you.

What do I bring for my appointment?

Please bring along your:

Outpatient Appointment Card (for existing patients)

  • Singapore NRIC, Birth Certificate/Health Booklet (if you are under 15) or Passport and Employment Pass / Work Permit / Dependent pass
  • Civil Service Card / Public Service Card (for civil servants and their dependants only)
  • Any other Medical Benefits Eligibility Card/documents

If this is your first visit, please bring along the items listed above and:

  • Referral letter from your polyclinic / other healthcare institutions/ private doctor
  • Medication, if any
  • X-ray and investigation records (within the last 6 months), if any

Do I require Fasting a day before my appointment?

If you are undergoing investigative tests or medical treatments which require fasting, please do not eat or drink anything after 10.00pm the night before the appointment.
