The TC prepares waitlisted patients to remain medically ready, coordinates essential tests and clinical assessment, and support potential living donors through their evaluations and logistics. Working closely with doctors, nurses, and allied health professionals, the TC ensures timely care, informed consent, clear communication at every stage. Following transplant, the TC oversees follow-up appointments, reviews lab results and medications, and provides ongoing education to help recipients maintain long-term health. 2.      LiverThe Liver Transplant Coordinator (TC) plays a critical role in managing the entire transplant journey, ensuring safe, timely, and patient-centered care. This includes coordinating multidisciplinary evaluations and clinics, monitoring eligibility and activating urgent liver offers, managing donor and recipient workflows for both deceased and living donor transplants, and tracking clinical progress through labs and imaging. The role also focuses on developing efficient workflows, providing education and emotional support to patients and caregivers, and maintaining seamless communication across medical and allied health teams to optimize outcomes.

3.      HaematologyTransplant Coordinators (TCs) support patients and clinical teams throughout the transplant journey, ensuring smooth, timely, and well-coordinated care. They manage pre-transplant workups, donor matching, scheduling, and patient education, while serving as the key point of contact for patients and families. With strong organisation and communication skills, they help streamline processes, uphold clinical standards, and contribute to positive patient outcomes.

4.      Skin BankTissue Transplant Coordinator (TC) serves as the crucial link between grieving families and life-saving donation for the purpose of tissue transplantation. The role combines clinical expertise with exceptional interpersonal skills.
