Aims

  • To help discharged SGH patients recover well in the community and prevent further hospital readmissions
  • Helping elderly in Singapore to experience the protection, love and care of a family by bringing the elderly (older generation), working adults (middle generation) and youth (younger generation) together as a "family unit", where there is mutual learning, understanding and care

Who can Volunteer?

  • Anyone 17 years old and above    
  • Volunteers are classified into 2 categories:
    • Healthcare Professional Volunteers: Doctor / Nurse / Allied Health Professional
    • Non-Healthcare Professional Volunteers: Students and working professionals not trained in medicine, nursing and allied health

What do I need to do?

Healthcare Professional Volunteer

  • Work in a multidisciplinary team to provide medical care for patients under Hospital to Home (H2H) & Community Nursing programmes through home visits

  • Guide non-healthcare professional volunteers in the team to understand the patient’s health condition (training is provided)

Non-Healthcare Professional Volunteer

  • Learn basic healthcare skills (training is provided) and apply them during home visits

Commitment: 1 home visit per month for 1 year

Location: Beneficiary’s home

Intake period

June - August 2022

Registrants will be notified of their registration outcome via email or text
