Clinical Networks' partnership with GPs started 19 years ago with the Delivering On Target (DOT) Right-Siting programme. The team works with selected SGH departments and Specialty Centres such as the National Heart Centre Singapore and National Cancer Centre Singapore to right-site patients with stable chronic conditions from Specialist Outpatient Clinics (SOCs) to DOT GP clinics for continued care. A dedicated team of Right-Siting Officers (RSOs) provides follow-up support, acting as the key liaison between patients, GPs, allied health professionals and specialists.
Over the years, Clinical Networks team has built trust and rapport with island-wide GPs.
In 2018, the SingHealth DOT Primary Care Network (DOT PCN) was launched. Since the past 5 years, the team has been working closely with the GPs and their clinic staff to better manage their chronic disease patients by supporting their clinical outcome tracking, provision of ancillary services and organising peer review learning sessions for upskilling.
