How to complete PROMs

Find out the different ways you can complete PROMs

Synonym(s):

Via SMS/Text

An SMS (text) with the link to the PROM questionnaires (e.g. FormSG) will be sent to the patient’s registered mobile phone number. See example on the right.

 

3 Easy Steps

Click the link in your SMS.

Complete the questions.

 

Hit submit once you’re done.

Beware of SPAM!

All digital PROMs (FormSG, etc.) are conducted on secure platforms and all patients’ responses will be kept strictly confidential for clinical purposes.

You will receive SMS or text messages from sender ID '77551', 'SingHealth' or other official sender IDs that your clinical team will inform you about in advance.

When will patients receive the PROMs SMS / text?

  • Before consultations
  • During hospital admission, OR
  • Post-discharge at home. 

Please refer to the instructions in the SMS sent.

 

 

Via Face-to-Face Interaction

Alternatively, a trained clinic staff may ask the patient the required PROM questions. 

This may take place at:

  • Clinic waiting areas,
  • Bedsides, or
  • Dedicated assessment rooms

The answers will then be recorded into the system for your care team to view the scores.

 

 

 

Via Phone Call

Patients may also receive follow-up calls post-discharge to answer PROM questions verbally. The liaison officer will then record patients’ answers into the system.


Data Protection

The privacy and data security are of utmost importance to us at SGH.

When our patients complete their PROMs, they can be assured that all information collected is treated with strict confidentiality and protected in accordance with Singapore's Personal Data Protection Act (PDPA).

The responses are securely stored in our encrypted database, accessible only to authorised healthcare professionals directly involved in their care. We use this information solely to improve patients’ treatment outcomes and enhance our overall quality of care. SGH does not share patients’ personal data with third parties without their explicit consent, except where required by law.
