The SGH Medical Social Services Community Care Team (MSS CCT) provides comprehensive support to patients and families throughout their healthcare journey. We facilitate seamless transitions between hospital and community-based services, ensuring continuity of care beyond discharge and empowering patients to maintain their well-being in their communities.
Meeting to facilitate care planning
We assist patients in making a smooth transition from the hospital back to the community that results in the continuity of care and efficient use of hospital and community resources. This can include referrals to community case management services, home help and home care services, day care centres, nursing homes and active ageing centres.
In addition to formal assistance schemes such as MediFund, we link needy, eligible patients with external resources to meet their treatment costs, and financial and practical needs arising from their illnesses.
We conduct home visits to establish patients’ coping capacity in their home environment, linking them with other community resources where necessary. Home visits also allow us to gain a better understanding of patients’ family dynamics and financial situation, hence facilitating us in our case assessment and intervention.
We offer counselling—managing issues from illness coping, illness anxiety, grief and loss, bereavement, and other emotional distress.
We are competent in Applied Suicide Skills & Intervention Training (ASIST) of LivingWorks (Calgary, Canada) Suicide and can assess and support patients presenting with suicide risk. We also provide risk assessment, safety planning and emotional support for victims of family violence, including domestic violence, child abuse and elder abuse.
We provide grief and bereavement support to families whose loved ones died of sudden or traumatic deaths.
The MSS CCT also network and collaborate with various partners including active ageing centres, community case management services, and home care providers to offer tailored support that extends beyond the hospital walls.
Talk on Understanding Community Resources for Dementia Patients and Caregivers at Jalan Batu Residents’ Network
We conduct training and talks in healthcare-related topics, such as in care and financial resources to community partners. We are also able to tailor and curate these talks to residents residing within our SGH Precincts of Care.
We are also keen to work in collaboration with our partners to co-develop strategies/interventions that enhance access to care and health-seeking behaviours for socially vulnerable population (e.g. needy, socially isolated, etc.) residing within the SGH Precincts of Care.
