Clinical Networks' partnership with GPs started 19 years ago with the Delivering On Target (DOT) Right-Siting programme. The team works with selected SGH departments and Specialty Centres such as the National Heart Centre Singapore and National Cancer Centre Singapore to right-site patients with stable chronic conditions from Specialist Outpatient Clinics (SOCs) to DOT GP clinics for continued care. A dedicated team of Right-Siting Officers (RSOs) provides follow-up support, acting as the key liaison between patients, GPs, allied health professionals and specialists.
