The role of nurses has evolved to meet the dynamic needs of an aging population. Community nursing has been established in Singapore with the aim to anchor population health and provide sustainable healthcare services beyond the hospital to the community. Community nurses provide health services to residents at the Community Nurse Posts (CNP) situated within the heartland residential estates.
Recognising the need for a paradigm shift in care for an ageing population, which brings about an increased prevalence of chronic conditions, it is an opportune for communtiy nurses to support the changing healthcare landscape and enhance upstream care for our residents in the community. The SingHealth South East (SE) Regional Health System (RHS) - led SGH community nursing service which is supported by the Ministry of Health (Singapore) came timely as one of the strategies to anchor population health and facilitate the shift beyond acute care to community care.
People with chronic pain faced potential treatment disruption during the COVID-19 pandemic in Singapore, as the focus of healthcare shifted. A model of rapid integration of a pain centre with community healthcare teams was implemented to care for vulnerable older patients with chronic pain and multiple comorbidities.

There is a growing trend of expanding Outpatient Parenteral Antibiotics Therapy (OPAT) services to the community as it is more cost-effective for both patients and healthcare organisations. At the Singapore General Hospital (SGH), frail patients requiring intravenous (IV) antibiotics often have to prolong their hospitalisation to complete the course of treatment. Community OPAT (CoPAT) service is a joint effort between SGH Community Nursing and Department of Infectious Diseases. With the new CoPAT service, patients have the options of receiving their IV antibiotics at home rather than receiving treatment at the hospital or outpatient clinic. This case study described the process of providing CoPAT services in the community, its challenges and learning points and the positive outcomes of cost saving and improved patient experience. Early preliminary screening of suitable patients and good communication link among hospitals and community teams are essential in ensuring its success.
Social prescribing connects people to assets within their community, to improve their social determinants of health. As many seniors are socially isolated amidst the COVID-19 pandemic, electronic social prescribing has emerged as a way to keep these vulnerable groups meaningfully engaged. SingHealth Community Hospitals seeks to collaborate with general practitioners (GPs) and other community partners to maximise this impact.

Therefore, this study aimed to explore the factors influencing vaccine uptake and the impact of COVID-19 on vaccine uptake among community-dwelling older adults in Singapore.
