Long-Term Oxygen Therapy Claims

Long-term oxygen therapy (LTOT) is used in patients with chronic obstructive pulmonary disease (COPD).

Patients who have rented devices for LTOT can submit their invoices for Medisave claims.

Before doing so, please prepare a softcopy of the following documents:

• Invoice from vendor
• Medical Claim Authorisation Form (For first-time patient)
• Undertaking to refund form

2. Download and complete these forms:
   - Inter-bank GIRO Form [295kb, PDF]
