About PHICO


About PHICO

Population Health and Integrated Care Office (PHICO) was established under Singapore General Hospital (SGH)Medical Board on 1 November 2018. PHICO was formed to oversee care integration initiatives of SGH and chart the overarching direction in population health by developing strategic models of care in the Southeast Communities of Care.

PHICO hopes to achieve its goals by forging synergistic partnerships with community service providers to co-create an empowered community where holistic care is delivered to enable every resident to live and age well.

The SouthEast ECoC map is a visual representation of the Population Health approach and vision by SGH in South East Singapore. The tree in the middle represents the ever-growing nature of our Southeast ECoC.

ECoC aims to achieving total well-being for residents through empowerment, across the life journey, allowing residents to Keep Well, Get Well and Live Well.


Our ABCDE Approach

Each alphabet represents the key drivers for ECoC, with various activities encompassing it. Each driver is interlinked with the other to facilitate and contribute to the growth of ECoC.


Activated Community HealthUP!

The Activated Community HealthUP! is at the heart of ECoC. We envision our residents to be well supported, engaging in active and healthy living, taking part in social activities and using technology in their everyday lives.

Resources and services required for residents to Get Well, Keep Well and Live Well are easily accessible in the community. PHICO works closely with our community partners on health programmes and activities through:

  • Active ageing
  • Social connectedness
  • Intergenerational bonding
  • Person-centred care
  • Volunteerism
  • Digital inclusivity

There is no "health" without well-being and self-care. We work towards normalising mental and emotional well-being across the ECoC. This is represented by the pathway from ECoC leading to the serene island.


Community Care & Social Services, Next-Gen Primary Care, SGH PHICO

These key components bridge the Activated Community HealthUP! to the SGH Campus behind. The well-connected routes linking the components of population health highlights the proximity and interconnectedness of various population health components to support an Activated Community. There is easy access for residents to move through the components based on their health and social needs.
