This talk presents the development of a Genomic Assessment Centre designed to integrate smart data tools, multidisciplinary expertise, and coordinated family screening into a seamless clinical pathway. Through unified digital workflows and collaborative care models, the centre enhances early FH detection and supports timely risk-reducing interventions. The presentation highlights key design principles, implementation challenges, and early outcomes, offering a scalable blueprint for genomic-enabled cardiovascular care.
