Comprehensive care for older patients in the hospital, is it possible?

​Hospitalisations can lead to acquisition or worsening of the geriatric syndromes. Geriatric syndromes consist of cognitive issues such as delirium, dementia, depression, incontinence, falls, malnutrition with weight loss or functional decline. Each hospital stay often causes the elderly to be in a worse state compared to when they were first admitted. This will cause a vicious cycle of re-admissions, further weight loss, functional decline, frailty and caregiver burden and eventually leads to increased cost, disabilities, caregiver burnout and institutionalisation.
