Care Integration and Coordination - A Case Management Approach (1 Nov 2018)

Singapore’s healthcare system is evolving rapidly. With advancement in medical technologies, treatment options, a rapidly emerging greying population and high expectations from consumers, it is vital to provide a seamless, integrated and coordinated care approach to support our clients ageing in place.  

Case Management was introduced in health care institutions in Singapore since the 1990s.  It is gaining recognition in view of scarce health resources, and the complexity of medical and social care needs.
 
In this session, the speaker will share on case management practices across the varied health care settings and the role and functions of case managers. This sharing will include the application of case management processes to support the client to live in his/her own home or at community facilities. Through the collaboration with health care Institutions and community care service providers, case management is therefore able to achieve a “wholistic” care approach for clients.
