Are you interested in helping patients recover and live well in the community?

Aims

  • To help discharged SGH patients recover well in the community and prevent further hospital readmissions
  • Helping elderly in Singapore to experience the protection, love and care of a family by bringing the elderly (older generation), working adults (middle generation) and youth (younger generation) together as a "family unit", where there is mutual learning, understanding and care
     

Who can Volunteer?

  • 18 years old and above
  • SGH Healthcare Professional Volunteers: Doctor/ Nurse/ Allied Health Professional
     

What do I need to do?

Healthcare Professional Volunteer (SGH only)
  • Work in a multidisciplinary team to provide medical care for patients under Hospital to Home (H2H) & Community Nursing programmes through home visits

  • Guide youth volunteers in the team to understand the patient's health condition (training is provided)

Commitment: 1 home visit & 1 tele-visit every month for 6 months
Location: Beneficiary’s home
