Patient Navigators

The role of the Patient Navigator (PN) was first introduced in SGH in 2014. There are 22 PNs in SGH working in various settings – Emergency Department, inpatient wards, disease-specific disciplines such as Chronic Obstructive Pulmonary Disease (COPD) and Vascular, and Community Hospital Referral.

Aim

The aim of the Patient Navigator is to facilitate comprehensive care coordination, timely discharge planning and seamless care transition from hospital to community.

Patient Navigators are resource persons for various hospital and community services. They provide guidance to staff, patients and caregivers on the recommended discharge disposition based on the patient’s care needs.
