Patient Navigators

Patient Navigators (PNs) assist patients with complex care needs, together with their caregivers, with their discharge plans to ensure safe, smooth, and timely discharge from hospital to the community.

First introduced in SGH in 2014, there are a total of 25 PNs in SGH, working in various settings such as the Emergency Department, inpatient ward, outpatient clinic and department specialty. 


Patient Navigators provide needed support to patients with complex care needs by:

Developing discharge care plan for patients

Working collaboratively with the multidisciplinary care teams to assess, plan, develop, and execute a comprehensive and appropriate discharge care plan for patients and their caregivers based on their care needs.  


Coordinating with stakeholders for seamless transition of patients

Liaison between the patients, caregivers/family members, healthcare teams, healthcare institutions, external agencies, and community partners, to facilitate smooth coordination, right siting, and seamless transition of patients across various settings.
