Geriatric Services Hub (GSH) focuses on treating geriatric syndromes and conditions such as frailty, through the development of a specialised care plan. A typical care plan comprises structured home based exercises, health coaching and referrals to health and social care services in the community.

GSH serves seniors living in the 6 Communities of Care - Bukit Merah, Tiong Bahru, Telok Blangah, Chinatown, Rochor and Kallang-Kampung Glam. Our multi-disciplinary team of healthcare professionals, comprising a Geriatrician, a Geriatric Nurse Clinician, a Geriatric Advanced Practice Nurse, a Physiotherapist, an Occupational Therapist, a Medical Social Worker and 5 Geriatric Community Nurses, work closely with partners to deliver quality care and support to enable seniors to age gracefully in the community.

Eligibility Criteria

• At least 65 years old
• Mildly Frail: Need help for daily activities, such as housework and taking medication.
• Moderately frail: Need help to bathe,  get dressed, or when outside of home.
• Severely frail: In stable medical condition but are completely dependent on others for most activities.
• Live in Bukit Merah, Tiong Bahru, Telok Blangah, Chinatown, Rochor and Kallang-Kampung Glam
