Conducted by SGH Community Nurses, and supported by other healthcare professionals from SingHealth institutions and community partners

Aim to leverage peer-motivation/ support and provide more disease-specific information focusing on self-management

Topic	Scope
Health Coaching	Include nurse counselling and sessions that focus on empowering residents to take charge of their own health.
Physical Activities and Falls	Include exercise demonstration, fall-risk assessment and fall prevention programme.
Disease-specific topics: Hypertension, Diabetes & Hyperlipidaemia	Focus on understanding disease conditions as well as ways to prevent and manage diseases.
