Dr Ng Wai May is the Deputy Director of Nursing and an Advanced Practice Nurse at National Neuroscience Institute. She has been practising neuroscience nursing for more than 20 years and has practised at various NNI campuses (TTSH, SGH and KTPH).  She is also a team member of Stroke Improvement Team set up by MOH since 2014 and has led and facilitated to establish the stroke system of care in Singapore.  She has also led her team to develop processes from Advanced Practice Nurses supporting hyperacute stroke treatment to stroke recovery program. Her APN service extends beyond acute hospital and into the community.  She is also actively involved leading quality improvement projects, Stroke Inter-Professional Education and research.

Stroke Recovery Programme - Care from Hyperacute Treatment to Home

This talk will discuss the importance and rationale for a seamless care approach for stroke patients. It will discuss improving practice processes in care areas with clear evidence-based guidelines as well as in areas with a paucity of data. The stroke recovery programme will be used as an example of showcase the benefits, hurdles and processes of developing platform across the continuum of care.
