Dr Deidre De Silva is a neurologist subspecialising in stroke. She is associate professor with Duke-NUS medical school and chair of the Ministry of Health Stroke Services Improvement team. Her interests in stroke care span from initial treatment in the emergency department to reintegration in the community, particularly looking at transitions across the journey and optimising care based on evidence-based guidelines.

This talk will discuss the importance and rationale for a seamless care approach for stroke patients. It will discuss improving practice processes in care areas with clear evidence-based guidelines as well as in areas with a paucity of data. The stroke recovery programme will be used as an example of showcase the benefits, hurdles and processes of developing platform across the continuum of care.
