If you are interested to pledge your skin or any other organs, you can either
1. Download and print the donor form, sign the donor form in the presence of two witnesses and send the form to:

a) Your name
b) Home address
c) Contact number (to confirm the receipt of Skin/Donor Form)
d) List number of donor forms you need
We will send you the number of donor forms you require.
Upon receipt of the donor form, kindly sign the donor form in the presence of two witnesses and mail the self-addressing form to the Organ Donor Registry at the Ministry of Health.
