DEPARTMENT OF FAMILY MEDICINE & CONTINUING CARE

Services

FMCC provide general medical care services to patients of the hospital regardless of age and gender.
FMCC conducts detail history taking and create a problem list. Patients are also assessed for co-morbidities  with a view to integrate and co-ordinate care.

Patients with multiple co-morbidities require care by multiple specialties.  Care co-ordination and integration is needed to minimize care fragmentation. In acute hospital inpatient services, patients often have multiple active medical problems other than the admitting diagnosis requiring multiple specialty consults. Similarly in the outpatient setting, they often have multiple follow-up appointments with many specialties. FMCC provides care integration and co-ordination to address the needs of a more holistic are of patients in the hospital.

Our special interest is to promote a patient centred approach to care with emphasis on co-ordination and integration between care providers. The scope of the consultation service includes: 

  • Diagnosis and management of medical problems, especially for patients co-morbidities across disciplines. 
  • General medical care of patients regardless of age and gender.
  • Discharge planning to optimize use of community and outpatient services.        

Inpatient Service
FMCC provide general inpatient care to 34 beds in Ward 63.
 
Outpatient Consultation Service
FMCC runs 10 sessions of specialist outpatient clinics, providing consultative service to patients with multiple co-morbidities, complex chronic diseases and undifferentiated health issues.

Pre-operative Evaluation Clinic
In collaboration with the Department of Anaesthesia & SIC, FMCC runs daily clinics in pre-operative evaluations of patients, focusing on optimizing medical conditions.

Health Assessment Centre
FMCC provides customized health assessment for patients at the Health Assessment Centre.
 
Home Medical Care for Sub-acute Patients
FMCC provides home based intermediate care to patients who are discharged from the hospital.  Transitional medical and nursing care is provided to patients at their home after discharge.  The service help to stabilize patients in the community until they are right sited to community care providers.
 
Training in Family Medicine and Hospital Medicine
FMCC provide undergraduate training to medical students in family medicine and general medicine.  FMCC is responsible for the Family Medicine Clerkship of the Duke-NUS (Graduate Medical School).

FMCC provides postgraduate training to family physicians preparing for basic and advanced family medicine training in collaboration with the College of Family Physicians Singapore.

FMCC provides training in hospital medicine to meet the need of physician providing general medical care to inpatients.  
 
Available Support Services
FMCC is supported by specialists from the other departments in the hospital.  The FMCC works closely with various support services including Medical Social Work, Physiotherapy, Occupational Therapy and Speech Therapy to provide multidisciplinary care.
