Request for Medical Information 

Need a medical report from the doctors? Need a copy of your x-ray? Need a copy of your investigation results? You can make your request online.

All you have to do is complete the patient consent form, make the payment and click the submit button. Thereafter, our Medical Report Office will process your request and contact you when your report is ready.

As a general rule, the time frame for completion of medical reports is between 3 to 4 weeks from time of request. Requests for duplication of investigation results will be completed within 2 weeks of receipt at HIMS.

You may either make one trip to the hospital to sign the consent form when you collect your report or mail in your duly completed consent form. A fee will be charged for request of the medical reports and x-rays films; and an administration fee of $5.25 will be charged for request for any investigation results.

Consent for Release of Medical Information

Due to legal requirements, only the patient can request for his medical report. Therefore patient's consent is necessary for the release of medical information.

For patients below 21 years of age, the consent of patient’s parent is required. If the patient is deceased/incompetent, consent is required from the appointed Administrator of Estate. If an Administrator of Estate has not been appointed, a separate Letter of Undertaking has to be completed by all family members of the patient. Copies of the death certificate and other supporting documents e.g. marriage certificate, birth certificate, and letters of administration / probate, as proof of your relationship to the deceased must be submitted together with the consent form.
