Discharge Your doctor will inform you when you are ready for discharge. It is advisable that you make arrangements with a family member or friend to accompany you home on the day of discharge.
A discharge certificate will be given to you before you leave the ward. If you need a medical certificate, please inform the doctor or nurse in advance so that it can be prepared before you leave. If medication is required, it will be dispensed to you at your bedside or you can collect them from the Satellite Pharmacy located nearest to your ward. Please ask the ward staff for specific instructions. Check Out Time The Hospital’s check-out time is 1.00pm. For check-outs after 1.00pm, an additional day’s rate will be chargeable. If you are waiting for someone to take you home, you may wish to wait at the Day Room. There are no additional charges for use of the Day Room. Follow-Up Care and Appointment Before you leave the ward, you will be advised on how to take care of yourself during your recovery.
The ward staff may give you an appointment for follow-up at the Specialist Outpatient Clinic. Should you need to reschedule your outpatient appointment after your discharge, please call our appointments line, at Tel: 6321 4377, or fill up the on-line SOC Appointment Form.

Medical Report If you require a copy of your medical report to claim from your insurance company, you may submit your insurance form or apply for a medical report at the Admission Office. Please note that there is an administrative charge for the application.

Discharge Planning Unit : CARELINK As part of our total patient care service, the Hospital's CARELINK Unit helps patients and their families plan for continuing care after discharge. As a one-stop service centre, CARELINK’s services include assistance with admission to hospices, nursing homes or community hospitals and arrangements for community home nursing care and rehabilitative services. CARELINK can also arrange for purchase or rental of medical/nursing supplies and appliances such as wheelchairs, commodes, oxygen concentrator, etc.
The CARELINK office is located at Block 6 Level 1 and operates during office hours which are as follows:
