In recognition that family physicians are the stalwart of public healthcare, the SGH "Partners In Care" is an initiative to engender stronger and more extensive co-operation between this hospital and the family physicians. Its programmes are pivoted on the belief that certain disease conditions are better and more efficiently treated when family physicians and specialists co-manage such patients. Instead of dealing with single episodes of care, family physicians and specialists co-operate to define a comprehensive care management plan for the patient.
Family physicians keen to be a partner in these Shared Care Programmes are invited to a series of Continuing Medical Education (CME) lectures and workshops. These CME sessions will focus on introducing the components for co-management of patients, such as:
- The role and involvement of the family physician
- When to refer a patient to a specialist
- Which patients are suitable for Shared Care
- Hospital management and discharge criteria
The Shared Care Programmes will eventually include of the following:
- Referral Criteria
- Defined care protocols for the family physician and the specialist
- Discharge criteria
- Indications for Re-referral to the Specialist
- Shared Patient Records that capture the necessary information for each patient episode
